Unike Support Services is a certified medicaid waiver, Support Coordination Agency. We assist and provide quality services for mentally and physically disabled individuals. Our responsibility is to be the liaison for the State of Florida and inform the client and their families on all the documentation needed to better integrate the consumer.

For quality assurance, a support plan will be created in order to give the Waiver Support Coordinator the knowledge and prowess to assess the proper services required for the particular individual; this includes:

1.) Medical history

2.) Personal goals

3.) Current living situation

Our Waiver Support Coordinators are responsible for supporting individual's self-direction, working creatively to meet their needs, and monitoring the individual's health and well being.

We are also responsible for promoting these individuals to be competitively employed based on their interests, talents, and abilities. Our goal is to provide and educate families in the necessary information needed to accommodate and maintain a well balanced individual with any special needs.
